Local Test Document Issuance Procedures: (A range of test materials will be issued to the Test Administrator according to the number of students listed on the test roster.)

Test Administrator will assign test documents using the class roster and test document roster to issue to students by test document number.

Test Administrator will write the name of each student next to an assigned test document number within the range of his/her assigned documents.

Erasure Analysis Procedure: (Excessive erasures, especially those that are wrong-to-right, are a matter of extreme concern.)
At anytime a test answer document is detected as excessive wrong-to-right answer erasures the procedure will be as follows:

Test Administrator will document, in writing, any excessive erasures by students during test monitoring. Written notes will be handed in to the site coordinator.

Test Administrator will notify the School Test Coordinator and the School Coordinator will notify the Principal and District Coordinator of excessive erasures.
-
The District Coordinator will convene a committee at the school building level, which will include the Principal, Test Coordinator, Test Administrator and Proctors, to conduct an investigation and write an explanation of findings, which will include written statements from everyone involved. The information will be kept on file.

After an investigation at the school building level, the committee will submit a report of findings to the district. If necessary, a district level committee will be formed consisting of the Superintendent, District Coordinator, and Assistant Superintendents. They will (if necessary) report their findings to the State Department.

There will be no further action for Test Administrators who report and document any excessive erasures during monitoring of tests.

If there are any excessive erasure irregularities that were not reported by Test Administrator the discipline procedures are as follows:

Test Administrator will be given due process
-
A district level committee will investigate
-
Depending on reason and severity of the irregularity, disciplinary action will follow board-adopted policy as per the contract.

C.

Student - Missing Test Booklet: (Booklet issued to student, according to Test Administrator's documentation, but not returned by student):
Following the Parish-wide procedure using the Security Checklist indicating booklet numbers, and sign in/out documentation for teacher and students, when a test booklet is determined to be missing, the following procedures will be implemented:

Test Administrator will immediately inform School Test Coordinator and Principal.

Students remain in class while the above team investigates/searches the class.
-
A written statement is taken by the teacher.
-
The School's Test Coordinator and Principal will interview student. (Student will be afforded due process)
-
The parent/guardian is to be notified by the end of the school day to schedule a conference.
-
Test Administrator will submit a written statement to School Test Coordinator, Principal, and District Test Coordinator.
-
Student's test score is voided and the school will follow the parish handbook "Code of Student Conduct' policy regarding discipline for a serious offense.

D.

Test Administrator - Missing Material: (Booklet issued by Test Coordinator, to Test Administrator, but not returned by Test Administrator):
Implement - Security Checklist Procedure (same as above)

Test Administrator, Test Coordinator and Principal will conduct a thorough search using the Security Checklist as documentation for reasonable justification.
-
A written summary of the occurrence will be sent to the School Test Coordinator, Principal, District Test Coordinator, and Superintendent.
-
A written report from the Principal, with recommendation to the Superintendent for disciplinary action, will be included with the summary report above.

Plagiarism:
At any time, if detected that a student has committed a testing irregularity by an act of plagiarism, the procedure will be as follows:
-
Documentation, in writing, by Test Administrator if incident is witnessed.

Report incident to School Test Coordinator, Principal, and District Test Coordinators.
-
Principal initiates investigation at the school building level.
-
District committee will conduct an investigation, if necessary.
-
Parent/guardian is notified to report to school for conference.
-
Voiding of the student's test, and school follows "Code of Student Conduct" regulations for disciplinary action.

Test Administrator's Breach of Monitoring Procedure:
If at anytime a Test Administrator is found not monitoring students during test (monitoring - walking around observing students on task behavior, and going ahead or going back on sub-tests) the procedure will be as follows:

Test Coordinator and Proctor/Observer will provide documentation in writing.
-
Report forwarded to Principal and District Test Coordinator.
-
Investigation will proceed at the school building level by the Principal.
-
Investigation will continue by District Committee, if necessary.
-
A written report with a request for disciplinary action according to Parish Policy is by the Principal to the Superintendent.

Test Administrator/Administrative Irregularities:
(Any action/activity committed that jeopardizes the integrity of the testing process.)
-
Refusal by Test Administrator to attend required testing in-service.
-
Test Administrator not becoming familiar with testing procedures through reading Test Manual.
-
Test Administrator/Administration not following Testing Time Lines.
-
Test Administrator not reading required script during testing.
-
Test Administrator intentionally giving verbal/non-verbal cues to students.
-
Test Administrator using unauthorized testing materials in classroom (dictionary, thesaurus, calculators, wall charts, etc.)
-
Improper monitoring by inactivity (sitting) during testing by Test Administrator.
-
Incomplete classroom coding information on required teacher input section of test documents.
-
Neglecting to document observed irregularities during testing.
(If any of the above occurs, the following procedures will be initiated):

The School Test Coordinator, or person observing, will document in writing to the Principal that negligent behavior has been observed regarding the Test Administrator.
-
The Principal will investigate and issue a report in writing to the District Test Coordinator and Superintendent, which will be written notification of negligence in testing security on the part of Test Administrator
-
Principal informs Superintendent in writing of negligence, for possible disciplinary action in breach of test security.

H.
|
Investigating, Missing Secure Materials:
Test Coordinator should immediately:
-
Double-check location of secured testing materials and check answer document and test booklet against test security sheet verifying test material is missing.
-
Notify Principal, Student, and Test Administrator, all students should remain in class until test document is found or officially determined unable to be found.
-
Investigate materials issued to the Test Administrator in order to determine whether document was used/unused.
-
Contact District Test Coordinator with information collected resulting from the investigation.

Facilitating, Cheating:

Due process afforded for both student and Test Administrator by Principal.

Parental conference is scheduled as soon as possible to inform parent/guardian.

Discipline of Student by Principal according to the SJBP Code of Student Conduct.

Disciplinary action of Test Administrator by Superintendent according to Board Policy.

Unused Booklet not returned by Test Administrator:

Test Coordinator checks for all documentation to verify missing materials.

Test Coordinator reports missing materials to Principal. Due process according to Union contract for Test Administrator by Principal.

Written documentation by Principal presented to Superintendent who reviews for disciplinary action.

State level representative notified of local action taken to handle test security issue.
